Use this form to record one alerting matrix row. Submit once per signal and repeat to build a complete matrix. The form captures routing, context, and template details so signals reliably reach an owner with clear next steps. After collecting multiple entries you can export them or combine into site-specific toolkits.

Name the observable event or metric (e.g., 'Line OEE < 60%', 'Order Delay > 48h', 'High CPU').
Briefly describe why this signal matters and what it indicates.
Choose the level of operational impact.
Specify the roles responsible for action (e.g., 'Line Supervisor; Maintenance Technician; Ops Manager'). Use role names rather than individual people where possible.
Select all channels to be used for this alert. Prefer channels that match the recipient's workflow to reduce ignored alerts.
Describe the immediate remedial or investigative action expected of the recipient (e.g., 'Acknowledge and investigate within SLA; if confirmed, execute runbook step 1').
Specify response time (e.g., '15 minutes', '4 hours') and whether SLA varies by severity or shift.
Who is notified if the initial recipient doesn't respond? Include time windows, next contacts, and automated escalation steps.
Rules to reduce noise (e.g., 'suppress duplicate alerts for same asset within 10 minutes', 'group by incident ID', 'only notify once per incident').
Person or role responsible for root-cause review and follow-up actions (e.g., 'Quality Lead').
Decide whether the system should attempt remediation or require human action to prevent unsafe automation.
Provide a template or link to on-call schedule (e.g., 'Weekdays 08:00-17:00: Line Supervisor; Nights: On-call Technician'). Use role + contact rather than personal emails when possible.
Provide a short alert message template including essentials: signal, affected asset, severity, timestamp, short context, and a direct link to dashboard or runbook. Example: 'ALERT: Line A OEE < 60% (critical). Asset: Line A. Time: {{ts}}. See: <link>.'
Paste URLs or identifiers for runbooks, dashboard panels, ticket templates, or automation scripts referenced by this alert.
Any other implementation notes, monitoring IDs, or configuration fields (e.g., metric path, environment, cluster).
