Use this checklist to confirm that a clinical AI model is ready for safe deployment into live workflows. The checklist captures ownership, evidence, controls, monitoring, and rollback planning so teams can make an informed go/no-go decision that protects patients and clinician trust. Answer each item, add concise notes where helpful, and record an overall readiness rating and decision at the end. Attach supporting documents (validation reports, training decks, runbooks) to the project record referenced in your CDS & AI Playbook.

How to use: One assessor (or small team) should complete this form. Mark Yes/No/Not applicable for each item and summarize evidence or next steps in the notes fields. Use the risk and readiness scales to prioritize remaining work.

Evidence should include dataset descriptions, performance metrics vs. clinical standard, and shadow-mode results where model outputs were not acted on.
Includes quick reference, interpretation guidance, common pitfalls, and escalation instructions for clinicians.
Provide URLs, file names, or where materials are stored. Attach files to the project record if available.
Plan should specify metrics, alert thresholds, data sampling cadence, and responsible owners.
Procedures must include who can pause/rollback, how to notify clinicians, and where to record incidents.
Includes data minimization, PHI handling, logging, access controls, and penetration/privacy testing.
