Use this worksheet to design, pilot, and document a simple Kanban system for consumable medical supplies. The form guides you through selecting pilot items, mapping usage and reorder points, defining Kanban card contents and placement, calculating suggested reorder quantities, assigning roles and daily checks, and capturing metrics you can track over time. Describe where each pilot item is used/stored, who reorders it today, and any current reorder triggers or par levels.

Where will the Kanban card and replenishment bin live? Choose the option that best fits each pilot item.
Only fill if you selected Other above.
Average units used per day for the pilot item. Use historical pull or par consumption data where available.
Typical time from reorder to delivery. For internal replenishment (central supply → unit) enter internal lead time.
Number of days of additional cover to reduce stockout risk. Start small (1–3 days) for a pilot and adjust based on variability.
Minimum orderable quantity or pack size. We recommend using this when converting calculated quantity to whole packs.
Suggested formula: avg daily usage * (lead time days + safety stock days). Convert to whole packs by rounding up to nearest multiple of pack size. Enter the final chosen quantity here.
Who performs the daily visual check, who pulls the Kanban, who places the replenishment order, and escalation steps for shortages.
Select metrics you will record to evaluate pilot success.
