Use this checklist when you notice early signs of disagreement, tension, or relationship strain. The form helps you record what you observed, choose appropriate immediate actions, and create a short, testable action plan with follow-up. Keep records factual and focused on behavior and impact rather than judgement.

Quick scripts (copy and adapt)

Private check-in: "I noticed you were quieter in the meeting and a couple of commitments slipped. I value your perspective — can we talk for 10 minutes so I can understand what’s going on and how I can help?"

Restorative conversation opener: "I want to understand what happened and how we can make this right. Can we talk about what each of us experienced and what we need to move forward?"

Interest-based negotiation starter: "We have different priorities here. Can we list our top concerns and then look for options that address both sets of needs?"
