Purpose

This short survey and worksheet helps teams quickly surface where trust supports or blocks performance across four practical dimensions: clarity (shared understanding), competence (reliable capability), care (mutual respect and intent), and consistency (predictable follow-through). Completing the 12 quick items and the one-page worksheet gives you evidence-based direction for one or two small experiments to improve team functioning.

How to use

  1. Each person completes the 12 statements using the 1–5 scale.
  2. Compute the average for each dimension (three items per dimension). Use the thresholds below to get a Red/Amber/Green snapshot.
  3. Use the worksheet fields to record the most important observation, one or two small experiments, priority, owner, and target date for each dimension.
  4. Save the submission to create shared data your team can use to plan follow-up (this item stores responses). For team-wide insights, aggregate multiple submissions and look for patterns across roles and time.

Scoring guide (quick)

For each dimension, average the three item scores (range 1.0 to 5.0). Suggested thresholds: Green = average ≥ 4.0; Amber = 3.0–3.9; Red = < 3.0. Focus immediate action on any Red area, then Amber items with high impact.

Rapid experiment ideas (examples)

  • Clarity: 15-minute role & priority huddle; one-page goals doc shared before work.
  • Competence: pair-review a deliverable; short skills swap session.
  • Care: gratitude round-robin or micro-retrospective about interpersonal wins.
  • Consistency: agree to one meeting cadence change or a simple definition-of-done checklist.
