Leadership Habit Tracker — Weekly Template

Use this weekly tracker to practice and reinforce core leadership habits. Enter the week start date, list up to four habits to focus on, then mark which days you completed each habit. Add short daily reflections and a simple progress score. Every four weeks, complete the review prompts to notice patterns, remove friction, and set realistic adjustments.

Tips: pick small, observable actions (for example: 'Ask two clarifying questions in 1:1s', 'Coach for 10 minutes', 'Pause 1 minute before responding'). Be honest — the tracker is a learning tool, not a performance report.

Enter the date that begins this tracking week (ISO YYYY-MM-DD). Helps when comparing weeks.
Enter a concise habit to practice (default suggestions: Active listening, Short coaching conversations, Daily reflection, Delegation).
Check the days you completed Habit 1. Use consistent criteria (e.g., any practice lasting 2+ minutes).
Optional — enter a second habit to track.
Optional — a third habit to reinforce.
Optional — a fourth habit to reinforce.
Short note: what worked, what you noticed, or one adjustment. (Keep it 1–3 sentences.)
Enter the total number of habit-completions this week (sum of checked days across all habits). Useful if you want a simple tally for progress tracking.
1 = Not at all, 5 = Consistently. Use this to capture a quick sense of momentum.

What progress, patterns, or wins have you observed over the last four weeks?
What consistently got in the way? Be specific (time, meetings, unclear expectations, energy).
What practical changes will reduce friction or make the habit easier to perform? (e.g., schedule a 10-minute block, ask a teammate to coach you, set a reminder).
A single concise commitment for the next 4 weeks (e.g., 'Ask one coaching question in each 1:1', 'Pause and paraphrase at least once per meeting').
Optional: anything else you want to record about this week.
