This scorecard helps interviewers evaluate candidates against role competencies, collaboration skills, and team-fit indicators. Use it to make evidence-based recommendations, reduce subjective bias, and speed onboarding by prioritizing collaborative capability.
How to use this template
- Before the interview, review the role profile and agreed competency weights with the hiring team.
- For each competency, mark the candidate's rating and add a short note linking the rating to observed behavior or an answer.
- Flag any serious concerns in the Red Flags section immediately.
- After interviews, follow the recommended consensus decision workflow to reach a team recommendation.

Scoring Rubric (use a consistent scale)
- 5 — Excellent: Clear, repeated, and recent examples that match or exceed expectations.
- 4 — Strong: Good examples and clear capability; minor gaps are manageable.
- 3 — Adequate: Acceptable performance with some development needed.
- 2 — Weak: Incomplete examples; important gaps in experience or behavior.
- 1 — Poor / Not observed: Lacks evidence for the competency or shows counterproductive behavior.
Note: Use agreed weights for each competency. If no weights are provided, treat all competencies as equal.

- Hold a structured debrief (15–30 minutes) focused on evidence, not impressions. Start with areas of agreement, then discuss differences.
- Decide next steps: proceed to offer, additional interview, reference checks, or reject. Record the decision, required conditions, and owner.
- Document onboarding needs (if offer likely) so the candidate can ramp faster (training, mentoring, 30/60/90 goals).
