Role & Accountability Definition Template

Use this template to make a role concrete, useful, and easy to hand off. A clear role definition reduces duplication, prevents gaps, speeds onboarding, and helps teams make better decisions together. The form captures why the role exists, the top outcomes it owns, decision authorities, who the role works with, how and when handoffs happen, escalation expectations, key skills, and development needs.

How to use it: fill the key fields, keep the role purpose short, write outcomes as measurable or observable results, and be specific about decision boundaries. Attach or link to any SOPs, job descriptions, or org charts in the Additional notes field. Save the definition where the team can reference and update it regularly.

Example (brief): Role title: Product Owner. Purpose: Define customer-facing product priorities so the team delivers highest-value features. Top outcomes: 1) Roadmap aligned to customer value; 2) Backlog refined and prioritized for each sprint; 3) Stakeholder feedback loop operational. Decision rights: Approve backlog items up to $X scope; Consult engineering on feasibility; Inform stakeholders of release dates. Escalation: Escalate unresolved priority conflicts to the Delivery Lead within 48 hours.

Short, consistent title (for example: Product Owner; Service Manager; QA Lead).
Who currently fills the role, or the team/role group responsible. Use names, team names, or 'vacant' as appropriate.
Why this role exists and how it contributes to the team or organization. Keep to 1–3 sentences.
A measurable outcome or result this role owns. Include a metric or acceptance criteria where practical.
Another primary outcome owned by the role.
A third important outcome (optional).
Use Recommend / Approve (Accountable) / Consult / Inform. Add clarifying notes below for boundaries or thresholds.
Clarify decision boundaries, thresholds, monetary limits, or decisions shared with others. Example: 'Can approve vendor contracts under $10k; escalate above $10k.'
Teams, roles, or external stakeholders this role works with regularly and the nature of those interactions (e.g., 'Daily with engineering for backlog refinement; weekly with sales for feedback').
Describe when the role takes ownership or passes work to others (e.g., after deployment, handoff to operations). Include observable triggers where possible.
Who to escalate to, under what conditions, and expected response timeframe (e.g., 'Escalate to Department Head within 24 hours if SLA breach is likely').
Select the competencies critical to perform this role well. Use Development priorities below to plan growth.
Name any other competency not listed above.
Concrete learning actions, training, mentoring, or stretch assignments needed in the next 3–6 months. Be specific (courses, shadowing, projects).
Percent of a full-time role allocated to this responsibility (leave blank if not applicable). Enter a number between 0 and 100.
Specific measures used to judge success (examples: % on-time delivery, customer satisfaction score, defect rate). Prefer measurable indicators over vague statements.
Known risks, dependencies, policy constraints, or resource limitations that affect the role. Identifying these helps plan mitigation.
Who can cover this role, current readiness level, and what is required to hand over quickly. Useful for continuity planning.
