Use this planner to prepare, hold, and follow up on an interest‑based conflict conversation. The form guides preparation, the structured conversation phases, and aftercare. Save your responses to create a record you can update after check‑ins.

E.g., 'Schedule disagreement with Alex'
List names and roles (who will attend).
E.g., '2026-09-01, 10:00 AM, Zoom' or 'In person, conference room B'
State what happened, when, and who was involved. Stick to observable facts rather than interpretations.
Which shared values, goals, processes, or standards are at stake?
Describe a realistic successful result for the relationship and the work.
Safety, legal, policy, schedule, or other constraints the conversation must respect.
Draft a short opening: purpose, acknowledgment of the relationship, and a request to proceed. Example: 'I want to talk about X so we can keep working effectively together. Are you willing to hear my perspective and share yours?'
If 'No', propose when you'll meet instead.
Invite each person to tell their story. Note key points and concerns; avoid editing into your own interpretation here.
After someone speaks, write a brief paraphrase to check understanding. Aim for accuracy, not agreement.
Surface the needs, pressures, or goals driving each person's position.
Brainstorm possible solutions without committing. Encourage quantity and feasibility checks later.
Record specific actions, who owns them, and clear deadlines. Use measurable language.
Concrete, observable behaviors or signals (e.g., 'Weekly 15‑minute check‑in', 'Acknowledged changes to X process').
When and how will you follow up? E.g., 'Weekly for 4 weeks, then monthly for 3 months'.
Mark yes to confirm the agreement has been shared with everyone involved.
Use this area to record progress, missed commitments, and adjustments.
Reminders for the facilitator: stay curious, reflect feelings, keep time, avoid taking sides, and summarize agreements.
