When to use

Use this simple ADR for major cross-team or long-lived decisions where capturing context, options, rationale, owners, and a review plan will save time and reduce rework. It works well for technical, process, policy, and product decisions.

How to use

  • Fill the required fields at the time the decision is made.
  • Link or attach supporting artifacts (designs, tickets, test results).
  • Assign clear owners and a review date so decisions are revisited and validated.
  • Use a consistent Decision ID or tag so records are easy to find (example: ADR-2026-12).

Storage & discoverability

Store records in a central decision log or knowledge repository with tags for teams, systems, and topics. Make sure visibility settings match the confidentiality needs.

Short unique id such as ADR-2026-01. Leave blank to generate or assign later.
Clear concise title that you'll search for later (e.g., 'Auth token rotation strategy').
Date decision made (YYYY-MM-DD).
Why this decision matters. Describe the background, constraints, goals, affected systems, and stakeholders.
List meaningful alternatives considered. Number each option and include brief pros/cons. Link to detailed analysis when available.
State the chosen option and explain why it was selected over alternatives. Include criteria and evidence used in the decision.
Describe important trade-offs, known risks, and planned mitigations or monitoring approaches.
Name owner(s) responsible for implementation and follow-up. Use role and name where possible (e.g., Product Lead: Alex).
Current lifecycle state of the decision.
Date to review the decision and its outcomes (YYYY-MM-DD). Leave blank if not required.
Links to designs, tickets, PRs, meeting notes, spreadsheets, or other supporting documents.
Record next steps, acceptance criteria, metrics to evaluate success, and who will do what by when.
