Purpose

This form helps managers capture behaviorally anchored evidence before calibration so ratings are tied to observable actions, not recent events or impressions. Use one submission per employee per calibration period.

Performance bands and behavior anchors

  • Exceeds expectations: Consistently goes beyond role expectations. Examples: coaches peers, drives measurable improvements, voluntarily assumes additional responsibility with positive outcomes.
  • Meets expectations: Reliably performs core responsibilities to the required standard. Examples: completes work on time with acceptable quality, collaborates effectively, meets agreed goals.
  • Developing / Needs growth: Shows potential but inconsistent performance or gaps that reduce overall effectiveness. Examples: meets some targets but misses others, requires coaching, occasional quality or timeliness issues.
  • Unsatisfactory / Improvement required: Performance fails to meet required standards despite support. Examples: repeated missed deadlines, quality failures, conduct or safety concerns.

Calibration cadence & facilitation recommendations

Run calibration sessions quarterly or at natural business-cycle milestones. Limit each session to a manageable roster (for example, 20–30 employees) and use a neutral facilitator. Start by reviewing anonymized evidence summaries, surface rating patterns, discuss edge cases, and document agreed adjustments with rationale. Focus on observable behavior and documented impact rather than labels or impressions.

How to use this form

Provide clear, concise behavioral examples with dates, outcomes, and impact. Avoid opinions without evidence. Where possible include objective measures. This submission is intended to be shared in calibration sessions with peers and facilitators when agreed.
