This short, evidence-informed survey helps teams quickly surface where they need targeted support across three practical domains: trust, clarity (roles & goals), and capability. Use it to start a candid conversation, not to assign blame. Encourage honesty: the tool is most useful when team members answer thoughtfully and (if they prefer) anonymously.
How to use this template
- Ask the team to complete the survey in one sitting (5–10 minutes).
- Aggregate responses and compute domain averages (see scoring rules below).
- Run a 60-minute workshop to explore low-scoring areas and convert issues into 1–3 concrete improvement actions with owners and dates.
Anonymization guidance
The form lets individuals leave their name blank for anonymity. If you plan to collect identifiable info, state clearly why and who will see it. For psychologically safe responses, anonymize or aggregate results before sharing with the full team.
Scoring (quick traffic-light analysis)
Each question uses a 1–5 scale (1 = Strongly disagree, 5 = Strongly agree). Calculate the average score for each domain:
- Green: average >= 4.0 — keep doing this and look for ways to scale or sustain.
- Yellow: average 3.0–3.99 — moderate concern; explore root causes and quick wins.
- Red: average < 3.0 — immediate attention; run focused problem solving.
Recommended 60-minute facilitation script
- (5 min) Frame the purpose: improvement, not evaluation. Remind participants about anonymity and norms.
- (10 min) Share anonymized domain averages and a 1-slide highlight of top 3 low-scoring questions.
- (20 min) Break into small groups (2–3 people). Each group chooses one low-scoring question to diagnose: what behaviors, processes, decisions, or constraints are causing this score?
- (15 min) Return to the room: each group offers one root cause and one proposed action (who, what, by when). Capture on a shared board.
- (10 min) Prioritize actions using simple criteria (impact, effort, owner). Assign owners and follow-up dates. Decide how progress will be measured and when to reconvene.
Follow-up & cadence
Share the anonymized results and the prioritized actions within 48 hours. Track the selected metrics weekly and repeat the survey quarterly or after significant change events.
