This short assessment helps teams quickly identify where to invest to turn operational data into trusted intelligence. Rate each area on a 1 (immature) to 5 (mature) scale, add brief evidence, and capture suggested quick wins and medium-term investments. The form stores your responses so teams can compare baselines, track progress, and create a prioritized roadmap (recommended for use with Deck 1103).

Rate how complete, connected, and accessible your operational data sources are. 1 = many manual gaps and siloed sources; 5 = comprehensive, well-integrated sources available to consumers.

List small, high-impact actions that increase trust or reduce noise (examples: tune alert thresholds, add owners to key dashboards, publish a short data dictionary).
List improvements that require coordination and resources (examples: implement data ingestion pipelines, create reconciliation jobs, deploy a standardized dashboard template).
Who should be accountable for data quality, alerts, and dashboard decisions? Include names, roles, or suggested councils.
