Tips: enter one corrective action per submission; paste links to evidence or document IDs in the evidence field; use YYYY-MM-DD for date fields so they can be parsed consistently in downstream tools.

Person or role responsible for implementing the action. Use a consistent naming convention (name or role/email).
Target date for implementation. Use YYYY-MM-DD so downstream tools can interpret the value.
Clear, observable criteria that will demonstrate the action resolved the root cause (what success looks like).
How the verification will be performed.
Paste URLs, document IDs, photo references, or system attachment IDs that prove the action was completed and effective.
Current state of the action.
Date verification was performed (YYYY-MM-DD).
Record whether the action was effective when verified.
Name of the person who confirms closure. Required when status is Closed.
Date closure was signed off (YYYY-MM-DD).
How often this action should be reviewed after closure to confirm sustained results. Enter whole number of months (e.g., 6).
Estimate the remaining risk that the issue will recur after this action.
Mark yes to trigger priority-driven escalation workflows or notifications in your organization.
Explain why escalation is required and what immediate steps are expected.
Owner can record updates, blockers, or next steps during implementation.
