5S Audit — Checklist

Use this interactive 5S audit to assess Sort, Set in Order, Shine, Standardize, and Sustain. For each area, provide a 0–5 rating, note observable evidence, propose corrective actions, assign owners where appropriate, and set a follow-up date. Saved responses are stored as JSON so teams can track trends, follow through on actions, and generate reports.

This checklist is intentionally practical: pick one area, walk the workspace, record facts, propose small improvements, and schedule a follow-up. Avoid perfectionism — the goal is measurable, repeatable improvement.
