This interactive baseline assessment helps your team quickly measure capability across core operational domains. Run as a 60–90 minute facilitated workshop: read each statement aloud, discuss briefly, and choose the score that best represents current, observable practice. Use the scoring rules below and capture brief evidence or examples when helpful.

Scoring guidance

Use a 0–5 scale: 0–2 = needs work, 3 = capable, 4–5 = mature. Base your score on current, demonstrated practice (not intent). After submission the system will calculate section averages, highlight areas of greatest gap, and produce suggested quick-win pilots.

Leadership & governance: Clear ownership exists for operational improvement with defined roles and accountability.

0=no ownership; 3=roles assigned and meeting cadence; 5=ownership embedded with KPIs and resourcing

Leaders visibly support improvement work (participation, resourcing, removing obstacles).

Improvement priorities are aligned to strategic and customer outcomes and communicated clearly.

Governance reviews use regular, time-boxed decision forums to prioritize improvement investments.

There are defined metrics and targets leaders use to assess operational performance and improvement progress.

Daily management & huddles: Regular shift/team huddles occur with focused agenda and visible follow-up of issues.

Huddle content is data-driven (safety incidents, quality defects, delivery, capacity) and time-boxed.

Huddles result in clear owners and timebound actions that are tracked to closure.

Visual management (boards, screens) makes performance and problem status obvious to the team.

Escalation paths and support are defined when teams cannot resolve issues at the huddle level.

Problem solving: The organization uses a consistent problem-solving method (eg, A3, 8D, PDCA) and trained facilitators.

Root causes are investigated beyond symptoms and corrective actions target systemic fixes.

Learnings from problem-solving are documented and shared to prevent recurrence elsewhere.

Improvement experiments use hypothesis, small pilots, and measurable success criteria.

There is a pipeline of prioritized problems with owners and review cadence for progress.

Standard work: Key tasks have documented standard work that is followed and accessible.

Standards are controlled and updated through change discipline when improvements are adopted.

Employees are trained and observed for competence against standard work.

Visual cues and tooling support consistent adherence to standards.

Data & dashboards: Core operational metrics are defined, owner-assigned, and updated timely.

Dashboards show trend, target, and context (eg, normal variation) to support decisions.

Data quality is checked and known limitations are visible to users.

Teams can access the data they need to run experiments and measure impact.

Performance reviews use the data to identify systemic opportunities, not just individuals.

Maintenance & reliability: Critical equipment has defined care routines and preventive schedules.

Maintenance work is prioritized based on risk and impact to operations.

The organization tracks reliability KPIs (eg, MTBF, MTTR) and acts on trends.

Maintenance knowledge (schematics, BOMs, troubleshooting steps) is available to technicians.

There is a feedback loop from operations to maintenance to reduce repeat failures.

Supply chain: Suppliers and inbound processes are measured and managed for quality and delivery.

There are contingency plans for critical supply risks and regular supplier performance reviews.

Inventory management balances flow with availability; stock and replenishment signals are reliable.

Inbound quality issues are captured, traced, and acted on with suppliers to prevent recurrence.

Supply chain improvements are part of the continuous improvement pipeline with owners.

Employee engagement: Frontline staff actively contribute ideas and participate in improvement work.

There are formal mechanisms to capture, evaluate, and implement employee suggestions.

Training programs exist to build problem-solving and CI skills across roles.
