Purpose

Use this template to create a clear, actionable preventive and predictive maintenance (PM) plan for a specific asset. Capture critical details so maintenance, operations, and reliability teams share one source of truth: what to do, when to do it, what to watch for, and how to escalate abnormal conditions.

How to use

  • Fill the asset header fields first.
  • Enter up to six maintenance tasks (add more copies if your site supports repeatable groups).
  • Provide spare parts, safety steps, and precise acceptance criteria so technicians can complete tasks consistently.
  • Include condition-based triggers and escalation steps for predictive maintenance.
  • Save the plan and link or import into your CMMS or maintenance dashboards.
Unique identifier used by your CMMS or asset registry.
Common name used by operators and technicians.
Physical location or area where the asset is installed.
Brief description of the asset, function, critical interfaces, and operating conditions.
Helps prioritize maintenance resources.
Describe the common or most important ways the asset fails and the typical consequences.
Explain how frequency was chosen (manufacturer guidance, run-to-failure risk, condition data, criticality).
The form includes fields for up to six tasks. If you need more, duplicate this form or request a repeatable task group capability.
Short task title (e.g., Lubricate gearbox).
Step-by-step or reference to the SOP. Include tools and inspection checks.
Choose the normal planned frequency.
Planned time to complete the task (hours). Use decimals for fractions.
Expected production impact when task is performed.
List part numbers and minimum stock to hold for this task.
E.g., electrician, millwright, certified inspector.
Include any lockout/tagout steps and PPE requirements.
E.g., vibration amplitude, oil sample ID, torque reading, visual checklist pass/fail.
Define pass/fail thresholds or measurement ranges that indicate the asset is acceptable.

Provide the quick operator-level checks to perform between scheduled tasks, and immediate actions for anomalies.
Select yes if vibration, oil analysis, thermography, sensor thresholds, or other condition monitoring is in place.
List sensors, metrics, thresholds, sampling frequency, and responsible monitoring role.
Specify formats, tags, sample IDs, frequencies, and where data should be stored (CMMS, historian, analytics).
Describe who is notified and the expected response time when a condition-based alert occurs.
Target percent of planned tasks completed on schedule.
Target mean time to repair.
Target mean time between failures.
Maximum acceptable breakdowns per reporting period (e.g., month).
How often this PM plan is reviewed for improvements.
