Operational Resilience Quickstart: Business Continuity Checklist

Use this quick interactive checklist to build a practical continuity plan for a single site, line, or team. The goal is a plan the frontline team can execute under stress: identify the truly critical processes, define acceptable recovery times, capture single points of failure and backups, set minimum staffing, and create a one-page recovery card for immediate use.

Fill the fields below. For critical processes, use one line per process in the suggested short CSV format so you can quickly export or copy information into local plans and the one-page recovery card. When you save this form, your responses are stored so you can update the plan and track readiness over time.

Tip: Keep the one-page recovery card short and actionable: incident name, owner, RTO/RPO, immediate actions, and who to call.

Name of the site, plant, line, or team this plan covers.
Who completed or owns this checklist.
Example: Packaging | 8 | 2 | 2 operators | Jane Smith This compact format keeps planning focused and easy to transfer into the recovery card.
List equipment, roles, systems, or suppliers whose failure would stop the process. Be specific (model, role, supplier name).
List supplier, contact, expected lead time, and any constraints (min order, certification).
List process -> minimum roles and counts (e.g., Packaging: 2 operators, 1 lead).
Start with incident lead, operations lead, site leader, safety, HR, and key customers. Use one contact per line.
Short actionable workarounds to keep critical processes running (what to do and who does it). Keep items one-line if possible.
List task -> owner -> contact. Example: Restore power -> Facilities Lead -> 555-1212.
Target date for a tabletop or live drill (YYYY-MM-DD).
Mark yes if assignments are made and leadership has approved a drill.
