Use this checklist to prepare, validate, and record planning details before scheduling maintenance work. The form helps ensure critical assets have parts and skills available, permits and safety checks are in place, and contingency steps exist to limit downtime. List part numbers, description, qty required and qty on hand. Include supplier lead times in days if not on hand.
Enter the longest single lead time among required parts, in days.
How often the preventive task normally occurs
Select all roles expected to participate
List required trades, certifications, or special training (e.g., electrician, instrument tech, confined-space trained). Indicate number of technicians needed.
Select permits that must be arranged before work begins
Short description of other permits (if applicable)
Confirm that permit applications, JSA/JHA, and pre-task briefings are complete.
Describe steps to reduce outage length or impact (e.g., temporary bypass, staged isolation, standby spare, perform in planned shutdown window). Include estimated mitigation time.
List immediate actions, responsible persons, and escalation path if the asset cannot be returned to service as scheduled.
Select KPIs the planner will monitor after execution
