Capture incident metadata, a stepwise timeline, artifacts to collect, witness statements, photographic guidance, causal-chain hypotheses and confidence, immediate containment actions, recommended countermeasures, and verification criteria. Save each evidence submission to link it to the RCA case. If you have more timeline entries or files than this form supports, add them in the 'Additional notes or links' field or submit supplementary records using the same Case/Incident ID.

Quick tips: use a unique RCA Case/Incident ID to link evidence records; prefer ISO dates (YYYY-MM-DD or YYYY-MM-DDTHH:MM); when taking photos include a scale or reference object, timestamp, and clear orientation.
