Purpose: Keep operations running when people change roles or leave by capturing the knowledge that matters most. Use this checklist to prioritize effort, assign owners, record evidence, and schedule verifications—without creating needless bureaucracy.

How to use: Work through each section with the role owner and successor(s). Be pragmatic: focus on high-impact items first. Save the submission so your organization retains a searchable record of the handoff, owners, and verification steps.

Name the role that is transitioning (e.g., Site Quality Lead).
Enter the expected last day or transition date (YYYY-MM-DD) if known.
Have you listed the most critical skills, recurring decisions, key artifacts (credentials, templates, contacts, system accesses) that the role performs?
Give 3–6 concise examples (e.g., 'Weekly supplier quality review', 'Calibration SOP owner', 'ERP batch release access').
Person responsible for capturing and storing the artifacts (name/email).
Estimate the operational impact to prioritize work.
Short, structured capture sessions (30–90 min) with templates produce higher-quality notes than ad-hoc interviews.
Select the template used for the capture session.
Date of the primary capture session (YYYY-MM-DD).
Provide a link or path to shared drive, wiki page, or repository.
Has the runbook or handoff card been created or updated to reflect current practice?

Assign a mentor and, where practical, schedule rotation or shadowing windows to transfer tacit knowledge.
List names and planned rotation/shadow dates (concise).
Validation through demonstration is more reliable than notes alone (checklists, observations, test tasks).

Record the current result.
Document what was observed, files produced, or tests passed (links encouraged).
Plan at least one check 30–90 days after handoff to catch gaps and questions.
