Purpose: Use this checklist to capture the essential context, access, contacts, and success criteria needed to move work safely from one person or role to another. Designed to preserve tacit knowledge, reduce repeated errors, and shorten ramp time.

How to use: Assign an owner, complete each section with concise, actionable items, attach or link artifacts where useful, set clear follow-up checkpoints, and obtain stakeholder sign-off. Save the form so the organization retains a searchable record for future onboarding and audits.

Describe the role, reporting lines, related roles, and key interfaces (internal teams, external partners).
List documents, playbooks, credentials, templates, runbooks, repository paths, and owners. Include links or storage locations when possible.
Concrete goals the incoming person should achieve in roughly 30, 60, and 90 days. Use measurable outcomes (e.g., 'own weekly report', 'reduce incident resolution time by X', 'complete safety training').
Who will shadow whom, duration, sessions to schedule, hands-on activities, and recordings to review.
How will you know the handoff worked? Include specific metrics, quality checks, and acceptance criteria for the receiving person and stakeholders.
List accounts, systems, roles to grant/revoke, who approves access, and any privileged credentials that require secure handover.
Primary contacts, backups, vendors, and escalation steps for common issues (include names, roles, phone/email, and hours).
Capture recent decisions, rationale, unresolved issues, known workarounds, and patterns that aren't obvious from documents alone.
Specify dates or intervals for check-ins (e.g., 2 weeks, 1 month), who will run them, and what will be reviewed.
List required courses, recertification deadlines, and required completion dates.
Document active problems, technical debt, customer issues, and immediate risks the incoming person should prioritize.
