Use this checklist during role transitions, reorganizations, and exits to make handoffs clear, verifiable, and easy to act on. Complete items with concise references (links, file names, ticket IDs) so successors and managers can find what they need quickly. Add notes where additional context helps and obtain manager sign-off when work is handed over.

Exact role name as used by the organization (useful for records and routing).
If known, enter the person taking on the role. Leave blank if TBD.
Use YYYY-MM-DD or local date format.
Short role description plus exceptions, recurring duties, and escalation points.
Provide direct links, filenames, or folder locations the successor should use.
List active projects, owners, next milestones, blockers, and any open tickets with IDs.
Include internal partners, vendors, clients, and typical escalation contacts.
List systems, account names or IDs, current access levels, and any delegated permissions the successor needs.
Do not revoke access needed for transition. Document revocations and effective dates.
Record decisions that shaped the current work, who made them, and why. This saves time and prevents repeated rework.
Include current metrics being watched, hypothesis, and expected end date.
Include canonical sources of truth (wiki pages, code repos, shared drives) and the primary owner for each.
Interview should cover priorities, outstanding risks, where to find docs, and immediate next steps.
Manager should confirm critical items before sign-off.
Flag legal or regulatory obligations that the successor must be aware of.
