Purpose

This audit identifies where organizational knowledge, decision-making, metrics, meetings, and learning loops break down so you can prioritize targeted remediation. Use evidence, not impressions: record artifacts, interview notes, and measured ratings for each area. The form stores responses so you can track follow-up and measure improvement over time.

How to use

  1. Scope the audit to a team, process, or decision area.
  2. Request artifacts listed below before interviews.
  3. Gather evidence, interview stakeholders, and rate each area on the 1–5 scales.
  4. Record top issues, assign severity and owners, and create clear remediation steps with target dates.

List inconsistent metrics, conflicting dashboards, or gaps in measurement that undermine decisions.
Consider agendas, roles, decision capture, timeboxing, and follow-up actions.
Rate how often meetings produce clear outcomes, assigned actions, and shared learning.
Observe recurring meeting problems: lack of agenda, poor attendance, no action capture, or siloed updates.
Does the organization capture lessons, test hypotheses, and update practices based on evidence?
Rate how consistently and effectively lessons are captured and applied.
Examples of experiments, retrospectives, root-cause follow-through, and evidence of changed practice.
Use these sample questions when interviewing stakeholders: 'What information do you need to make this decision?', 'Where do you look first for answers?', 'When did a recent decision go wrong and why?', 'What knowledge is missing when a new person starts?'.

Assign an overall severity for the set of top issues.
For each top issue, propose a concise action (e.g., 'Create decision record template', 'Map onboarding gaps').
Name or role who should own follow-up.
Target completion date or timeframe for remediation.
A short plan with milestones, who needs to be involved, and how success will be measured.
If yes, schedule owner and cadence in the remediation plan.
Record agreed next steps, meeting dates, or handoff details.
