Use this form to capture decisions clearly so teams move faster and so learning is preserved. Name who decides, who advises, and who contributes; record the rule, timeline, handoff, evidence, outcome, and lessons learned. Field help text contains short examples for product, operations, and strategic decisions.

Short unique id (example: PROD-UX-2026-01). Helpful for cross-references.
Concise summary of the decision (example: Choose A/B test metric for checkout).
What is included and excluded (example: applies to web checkout only; mobile is out of scope).
Name or role with authority to decide (example: VP Product or Product Council).
Comma-separated names or roles to consult (example: UX Lead, Data Science, Legal).
People providing analysis, implementation, or supporting input (example: Analytics, DevOps).
Person/role who must sign off if different from decider (example: COO).
How the decision will be made
Target decision date and key milestones (example: Decision by 2026-09-15; pilot Q4).
Who will execute and how the decision is handed off (example: PM to create tickets and assign owners).
Brief reasons and key evidence supporting the recommendation (data, experiments, user research).
URLs to docs, tickets, dashboards, or experiment results. Separate with commas.
Final decision and short description — filled after the decision is made.
Date final decision made (YYYY-MM-DD).
What worked, what to change next time. Capture after execution to make future decisions faster.
Suggested structure to reach the decision (example: pre-read + 15-min clarifying Qs + 20-min debate + decision + assign actions).
Who to escalate to if the decision stalls, new risks appear, or consensus fails.
IDs or links to related policies, roadmaps, requirements.
