Use this checklist to capture the key facts, access, runbooks, ramp plan, and acceptance for a role transition. Fill the fields, confirm the checklist items, and capture the acceptance signature so the transition is auditable and repeatable. Adapt fields to your team or role as needed.

Name the role or position being handed off (e.g., Site Maintenance Lead).
Person handing off the role. Include contact info if helpful.
Person taking on the role. Include contact info if helpful.
Use YYYY-MM-DD or a short date phrase (e.g., 2026-09-01 or 'week of Sep 6').
List systems, account names, roles/permissions, and any provisioning notes. Example: 'Jira - project-admin; AWS - read-only; Payroll - request via HR'.
Has the incoming owner been given and validated the required access?
If access is incomplete, list outstanding items and owners responsible for provisioning.
Link or list the most important runbooks the incoming owner must know and where they live (document paths, repo links, or shared drives).
Summarize active experiments, pilots, or work-in-progress that the new owner should monitor or continue. Include owners and expected checkpoints.
Point to decision records, rationale, and stakeholders for major recent choices the incoming owner should understand.
List weekly/monthly recurring items (e.g., 'Weekly ops review, monthly inventory reconciliation, quarterly audit prep').
List regular meetings the role attends and the main internal/external stakeholders to know. Include meeting cadence and purpose.
Name and contact of a mentor or buddy who will help the incoming owner in the first weeks.
Email or phone for the mentor/buddy.
High-level objectives or milestones for the first 30, 60, and 90 days. Useful headings: '30 days - Learn & stabilize; 60 days - Own & improve; 90 days - Lead & optimize.'
A short, concrete list of urgent priorities the incoming owner should address immediately after the handoff.
List key risks related to the role or current projects and recommended mitigations or contingency steps.
Describe what 'successful handoff' looks like (examples: 'All critical access granted and verified; incoming owner completed three shadowed shifts; acceptance signed').
Tick items to confirm they've been completed during the handoff.
Incoming owner selects whether they accept the handoff now.
Type the full name to record acceptance. For formal/legal signatures follow HR procedure.
Date of acceptance (YYYY-MM-DD).
Anything else worth capturing (links, attachments, next checkpoints).
