Use this form to record a single action arising from a meeting: what will be done, who owns it, when it is due, and how you'll follow up. Submit one action per entry. You can submit multiple entries during or after a meeting to build a reliable action list. Saved entries become structured data you can feed into reminders, huddle agendas, exports, or dashboards.

Tips: Keep action descriptions outcome‑focused (what success looks like). Use a consistent date format such as YYYY-MM-DD. If you plan experiments, tag them so learning can be tracked separately.

Describe the decision or task clearly. State the intended outcome (who, what, why).
Name or role of the person responsible. Use a team name for shared ownership.
Use YYYY-MM-DD, a short phrase (e.g., 'EOD Fri'), or the team's agreed format.
Optional: reference ID or URL for the decision record or meeting note (e.g., DR-2026-07-01).
Optional: mark this action as an experiment/pilot to collect learning intent and outcomes.
Track progress. Update as work proceeds.
After completion or during check-ins, capture what happened, blockers, and lessons learned.
Optional: helpful for sorting in reports.
Set a numeric reminder to trigger a follow-up n days before the due date. Requires reminder/notification integration to act on this value.
