Purpose: Capture the essential facts, decisions, outcomes, and reusable assets from client work so repeated work becomes easier, safer, and faster. Use each saved entry as a living memory item that people or AI can search, reuse, and act on.

How to use: Fill the fields after meaningful calls, milestones, or when a new repeatable decision emerges. Link to templates or playbooks rather than copying them. Tag consistently. Use the capture-after-call checklist below to make this a habit. Review and prune periodically.

YYYY-MM-DD or natural date. Helpful for pruning and timeline.
Name(s), role(s), and best contact method.
Choose the closest type. Helps with prioritizing reusable assets.
Industry, size, tech stack, primary goals, typical buyer, contract terms. Keep short bullets.
One-sentence summary of the core problem the client pays to solve.
Record decisions, who made them, and why. Include constraints, assumptions, and alternatives considered. This prevents revisiting the same debate.
Short note about how this decision changes deliverables, timelines, costs, or success criteria.
Specific results the client cares about and measurable indicators (KPIs, timelines, acceptance tests).
E.g., conversion rate target, uptime, revenue, number of leads, delivery SLA. Link to dashboards if available.
List deliverables that can be reused (checklists, email templates, report templates) and where they live. Prefer links.
Paste links to cloud folders, playbook pages, or template IDs so others can find them.
Names or links to playbook entries that should be updated or consulted.
Use consistent tags: industry:healthcare, product:webinars, contract:retainer, stack:wordpress. Examples below.
