Use this interactive supplier scorecard to evaluate potential and existing suppliers, guide onboarding, and track red flags. Enter measured metrics and complete the onboarding checklist. Saved responses build an operational record you can compare over time or across locations.

Scoring guidance: enter percentages (0–100) for On-time, Fill Rate, and Invoice Accuracy. Use the product quality and responsiveness scales (1–5). Enter complaint response time in hours. You can enter an Overall Score manually or use the suggested normalization guideline provided in the field help.

Include phone or email for escalation.
Percentage of deliveries arriving on the confirmed delivery window (0–100).
Percentage of ordered units actually delivered (0–100).
Percentage of invoices that match received goods and agreed prices (0–100).
1 = Poor, 3 = Acceptable, 5 = Excellent.

Optional. Suggested manual formula: normalize % metrics and scales to 0–100 and average. Example normalization: On-time% + Fill% + Invoice% + (ProductQuality*20) + (Responsiveness*20) — then divide by 5, subtract a small penalty for high complaint response hours if desired.
