How to use this scorecard

Use this interactive form consistently for every candidate for the role. Answer the five behavioral questions, rate each 1–5, add brief notes, run or record a trial-shift evaluation when applicable, call references, and record an overall recommendation. Scoring guidance: Minimum total score to pass: 18/25 (adjust to your local standard). Critical-fail rule: any score of 1 on the Allergen & Accuracy question should be treated as an automatic fail — mark the Critical Allergen Fail field accordingly. Strong behavioral answers and coachability can sometimes outweigh limited experience; record any conditioning (e.g., hire-with-training) in Recommendation and Notes. This form will be saved with the candidate record for later calibration and reporting.
