How to use this form

Fill the pre-shift section at the start of the shift. Make the hourly check one quick entry each hour (it only takes a minute) and use the debrief at shift close. Keep entries short — this is designed for busy leaders who need to stay on the floor.

Quick positive (in the moment)
“Nice touch—thanks for plating that consistently. That’s exactly what keeps guests coming back.”

Quick correction (in the moment)
“Hey—swap that garnish with the correct portion and run it. Thanks, I’ll cover tickets for a sec.”

Short private redirect
“I noticed the last three salads were underdressed. Let’s run the next two together so you can see the portion. I’ll check back in 15 minutes.”

Handling a guest complaint
“I’m sorry we missed the mark. Let me make this right—could I offer a replacement or a comp? I’ll get the manager involved if you prefer.”

When to escalate: If an issue repeats after two corrections, or it’s safety, legal, or major guest-impacting (allergen mishap, suspected foodborne illness, serious equipment failure, or repeated theft), escalate immediately.

Enter the date and shift start time (e.g., 2026-09-14 16:00).
Your name so managers know who submitted the log.
Use this to confirm you checked key readiness items at shift start.
Who’s expo, closer, POS backup, zone leads (one-line each).
If NO, list equipment issues in the next field and any immediate actions taken.
List faults, time noticed, and immediate fixes or workarounds. Leave blank if none.
Note proteins, sauces, garnishes you checked and any shortages. Example: 10 steaks prepped, hollandaise low.
Quick visual check for hazards, towels, sanitizer, and blocked paths.
Choose one focus to announce (e.g., speed, upsell special, plate quality).
Menus, reservation notes, stations set. Short notes for FOH readiness.
Enter the time of this hourly check (e.g., 18:00). Make one hourly entry each hour.
Spot-check recent plates (choose best fit).
If YES, note the blocking issue in hourly_blockers.
Short note: e.g., grill backlog, missing garnish, POS delay.
Quick sweep result for pass, expo, restrooms. If NO, note action taken.
One positive and one correction if given (example: praised quick expo; corrected portioning).
List items low or expected to run out soon (e.g., 3x chicken breasts left).
Approximate number of remakes during your shift. Helps track quality issues.
How many issues required escalation to manager/ops during this shift.
Number of times you called out specific positive behaviors.
Number of brief corrective coaching moments you used on shift.
If YES, use the escalation_message field to provide structured detail.
Use the template: Escalation: [issue]. When: [time]. Impact: [guest safety / service delay / stockout / equipment]. Immediate steps taken: [actions]. Requested support: [replacement part / manager decision / HR follow-up].
One or two wins to recognize. Short bullets are fine.
One specific problem to fix (who, what, when).
One small next step assigned before the next shift (e.g., retrain on portion, reorder item, check equipment). Include assigned person if known.
Record anything recurring so managers can track patterns (e.g., consistent sauce shortage every Friday).
