Use this checklist to make sanitation predictable, auditable, and inspection-ready. Complete the header information, mark tasks as completed, record ATP or visual verification values when used, note exceptions, and obtain supervisor verification. Guidance: record ATP readings as numeric RLU values if you use ATP; record pass/fail using the tolerance shown. Keep supporting records (invoices, service reports) attached to your location's sanitation folder.

If Yes, describe in the comments and notify manager immediately.
Describe any corrective actions taken or required, responsible person, and target completion date.
