Use maintenance records to count corrective actions in the last 12 months.
Typical downtime when this asset fails; used for impact assessment.
Manufacturer guidance or internal standard (e.g., 8 years).
Approximate percent = (asset age / expected life) * 100. Enter 0–100.
Select Yes if failure could cause immediate safety, legal, or major operational impact.
High-level CAPEX estimate used for budgeting.
Owner-assigned priority for replacement planning based on budget and strategy.
