Use this scorecard to create consistent, auditable supplier reviews. Enter category scores (0–10) and optionally adjust weights to reflect your priorities. A suggested default weighting is provided; calculate the weighted average as: sum(score_i × weight_i) / sum(weights). You may enter the calculated overall score or leave it blank and store the component scores for later analysis. Record observations and follow-up actions so reviews lead to measurable supplier improvement.
