Welcome
This form helps hiring teams evaluate candidates consistently and track your hiring funnel. Complete one submission per candidate (or per interview) and use the funnel snapshot fields to record a weekly or daily view of where candidates are in the process. Scoring uses a 1–5 scale where 1 = Poor, 3 = Meets expectations, 5 = Exceptional.
How to use
- Fill candidate details and role.
- Score each competency thoughtfully — explain scores in Notes.
- Choose a recommended action and, if offering, edit the Offer template below.
- Use the funnel snapshot at the bottom to record counts so you can spot bottlenecks across days or weeks.
Role-specific sample interview questions (copy or adapt)
Servers / Front of House
- Tell me about a time you turned an unhappy guest into a repeat customer.
- How do you prioritize during a rush?
- How do you handle special requests or allergies?
Line Cook / Kitchen
- Describe your process for maintaining food quality during a dinner rush.
- How do you follow recipes and manage portion consistency?
- Give an example of a time you fixed a quality problem before it reached a guest.
Manager / Supervisor
- How do you coach someone who’s consistently 5–10 minutes late?
- Tell me about a scheduling decision you made that improved service or reduced labor cost.
- How do you handle inventory shrinkage or supplier issues?
Bartender
- How do you upsell without making guests uncomfortable?
- Describe how you maintain speed and accuracy during a bar rush.
- How would you handle a guest who’s had too much to drink?
Scoring guidance: consider using the same interviewer for a role when possible, use notes to justify low or high scores, and compare candidates against the role's must-have skills first. At the end of the form you’ll find editable templates for offer and rejection messages you can copy into your hiring system.
