This interactive log is designed for one submission per waste event. Capture a clear estimate of how much was wasted, the reason, an approximate cost, and a suggested corrective action. Use the owner and follow-up fields to close the loop. Regularly review submissions to identify high-impact waste sources and run targeted reduction experiments.

Select the primary cause. Add extra detail in notes if needed.
Estimated cost of the wasted items in your local currency. Useful for prioritization.
Suggested change to prevent recurrence (order qty, training, recipe tweak, labeling, etc.).
Name or role responsible for acting on the suggestion.
