This interactive template helps teams make waste visible by capturing each loss event with consistent fields: what was lost, how much, why it happened, when and where it happened, who recorded it, and the estimated dollar impact. Fill one entry for each discrete waste event (plate waste, spoilage, over-production, theft, etc.). At the end of the week use the optional weekly summary fields to record totals, targets and planned corrective actions.

Tips: weigh items when practical. Estimate dollar impact using your recipe/portion cost per unit. Be honest — accurate data creates better improvement opportunities.

Describe the product or ingredient lost (e.g., ‘Caesar dressing’, ‘Burger patty #3’).
Enter the measured amount (use chosen unit). If exact weight isn't available, estimate consistently.
Optional — clarify when 'Other' is selected.
What was done (e.g., returned to shelf, notified manager, retrained staff, adjusted par).
Estimate using recipe/portion cost (enter numbers only, currency implied).
Any extra context (e.g., guest allergy, special order, supplier problem).
Fill only when submitting a weekly summary. Useful for tracking progress vs target.
Optional—use at weekly wrap-up to identify top cause.
