Use this form to record preventive maintenance tasks, capture key equipment details, prioritize spare parts and vendors, and create a log entry you can reuse. If you manage multiple pieces of equipment, submit one entry per scheduled task or spare-part update. Include dates in ISO format (YYYY-MM-DD) for best tracking. For bulk spare-part updates you can paste CSV or line-delimited lists into the Spare Parts field.

Enter the date the task was last completed (YYYY-MM-DD). Leave blank if this is the first entry.
Optional: enter an expected next due date (YYYY-MM-DD) to support scheduling and planning.
How critical is this equipment/task to operations? 1 = low impact, 5 = stops service / safety risk. Use this to prioritize spare parts and vendor SLAs.

List part number, description, qty on hand, minimum qty, and preferred vendor separated by commas or new lines. Example: PN1234, Thermostat, 2, min 1, ACME Parts.
Optional. Name of supplier or service vendor associated with this equipment or part.
Phone, email, account rep. Include any contract or SLA notes.
Average time to receive a spare part or schedule service.
Mark yes if this vendor is pre-approved or contracted for repairs/parts.
Add anything shift teams should know: lockout steps, PPE, serial numbers, or where spares are stored.
