Use this quick form for 3–5 minute coaching observations. Focus on one behavior, make notes specific and factual, end with a positive expectation, and give one clear action the associate will take before the next shift. Suggested micro-exercises below can be used when practicing.

Write brief, concrete observations (what you saw or heard), not interpretations. Example: 'Greeted guest after 40 seconds; smile; no upsell attempt.'
Name at least one specific thing the employee did well to reinforce positive behavior.
Limit this to one focused improvement the employee can act on before the next shift. Be specific and observable.
Define one concrete action the employee will do (e.g., 'Practice greeting script 3 times during 10-minute prep', 'Perform two plate checks on next two tickets').
Ask the employee to rate how confident they feel about completing the action (1 = not confident, 5 = very confident).
