Process Handoff Audit — Find the friction where work passes between people

Handoffs are a frequent source of delay, rework, and dropped responsibility. This audit helps teams inspect one process end-to-end, identify weak handoffs, and surface concrete fixes.

How to run the audit (45–75 minutes)

  1. Pick a process: Choose a process that routinely slows the team — e.g., feature delivery, onboarding a new customer, vendor procurement.
  2. Map the steps briefly: On a whiteboard or doc, list the major steps from trigger to completion (5–12 steps is ideal).
  3. For each handoff, answer three quick questions:
    • Is the receiver clearly identified? (Yes/No)
    • Are the required inputs documented and delivered reliably? (Yes/No)
    • Is there a measurable confirmation of receipt or acceptance? (Yes/No)
  4. Score it: Give each handoff 0–3 (0 = all No, 3 = all Yes). Lower scores indicate priority fixes.
  5. Root cause for low-scoring handoffs: For the worst two, ask: Is the role unclear, inputs inconsistent, timing misaligned, or tools missing?
  6. Select 2 countermeasures: Pick one short fix (e.g., clarify role or add a simple template) and one structural fix (e.g., change cadence or automate a notification). Assign owners and a 2-cycle trial period.

Sample audit questions you can copy

  • Who hands off to whom? (name the role or person)
  • What exactly is delivered at the handoff? (artifact, data, approval)
  • How often is the handoff timely? (always/often/sometimes/rarely)
  • When the receiver is missing information, how often does it cause rework? (never/rarely/often/always)
  • Is there an explicit acceptance step (receiver confirms or signs off)? (yes/no)

Small fixes that often work

  • One-line handoff template shared in the tool you already use (e.g., "Deliverable — Owner — Due — Acceptance Criteria").
  • A single acceptance check at the end of the handoff (e.g., receiver posts "accepted" in the ticket system).
  • Short coordination huddles when two teams cross frequently (10–15 minutes), not every day.
  • Automated notifications for critical handoffs tied to a ticket or status change.

Run this audit quarterly for processes that matter. The goal is to discover low-cost fixes that reduce rework and make responsibility visible.
