Quick audit checklist — 10 things to check during a short walk-through
- Is the work done where the procedure describes, or are people using workarounds?
- Are tools, labels, and instructions consistent and visible in the work area?
- Are handoffs clearly marked with an owner at each step?
- Do staff regularly explain short-cuts or ‘hacks’ they use to get work done?
- Are there recent near-miss reports or informal complaints matching what you see?
- Is required safety or quality equipment available and in working order?
- Does the physical layout encourage or discourage correct sequence of steps?
- Are data or records complete and up to date where decisions are made?
- Were there any recent exceptions that were not reported or logged?
- Can a short, low-cost test be designed to reduce this risk within days or weeks?
