  • Measure: Defects per hour or rework events across pilot vs control.
  • Escalate if: No improvement after two cycles—review tooling or training depth.

2. Variable incoming materials

  • Action: Add a quick acceptance check (simple go/no-go) and record failures.
  • Experiment: Sample 30 incoming items a week and log exceptions for 2 weeks.
  • Measure: % of lots failing acceptance and downstream defect correlation.
  • Escalate if: High supplier variance—engage procurement or request improved specs.
