Use this brief audit to identify high-risk areas and prioritize capture work. Answer each prompt and score it 1 (low) to 5 (high). Higher totals indicate more urgent need for capture.

Audit prompts

1. How many tasks are currently performed by only one person? (1 = none, 5 = many critical tasks)
2. How often do role changes or departures create urgent operational problems? (1 = rarely, 5 = frequently)
3. How many incidents or errors repeat because past fixes are not documented? (1 = none, 5 = many)
4. How discoverable are existing guidance and artifacts? (1 = easily discoverable, 5 = mostly hidden)
5. How frequently do new hires require extended one-on-one support to perform core tasks? (1 = rarely, 5 = frequently)

Scoring and interpretation

Add your scores. Total 5–15 = low priority, 16–20 = medium priority, 21–25 = high priority. For medium and high priority areas, start with a 30‑day sprint focused on the top 3 tasks.

Suggested next actions by priority

• High (21–25): Launch a capture sprint, schedule interviews, and assign a curator within 2 weeks.
• Medium (16–20): Capture the single most impactful task and pilot a handoff checklist in the next month.
• Low (5–15): Maintain lightweight capture for new projects and revisit the audit quarterly.
