Describe current performance using specific numbers where possible

List categories and top potential causes uncovered during brainstorming
Summarize the root cause(s) you will address and the evidence that supports them
List proposed fixes, owner for each, and planned test
How will you know the countermeasure worked? Include baseline and target values.
Key dates: test start, review, full rollout
How often will you check verification metrics?
Person responsible for driving the A3 to verification
After verification, note what changed, training required, and where to update standard work
