Weekly Huddle — Deep Review Template

Use the weekly huddle to move beyond immediate containment and into learning and prioritization. Treat it as the team’s short-term improvement board: a time to verify countermeasures, review trends, and reprioritize.

Suggested structure (45–60 minutes)

  1. Alignment (5 min): Confirm meeting aim and any time-sensitive updates from leadership or customers.
  2. Trend review (10–15 min): Show 2–8 KPI trends. For each metric, ask: what changed this week and why?
  3. Problem deep dive (15–20 min): Choose 1 recurring issue. Quickly review containment, current impact, suspected root causes, countermeasures in place, and evidence of effectiveness. Use lightweight A3 steps if helpful.
  4. Improvement pipeline (10 min): Review status of ongoing experiments, resource needs, and next priorities. Decide which experiments to continue, scale, or stop.
  5. Actions & verification (5 min): Confirm owners, deadlines, and how results will be measured next week.

Questions to guide the deep dive

  • When did this problem first appear? Is it new or recurring?
  • What containment did we apply, and did it hold?
  • What evidence links root causes to the observed effect?
  • What experiment or countermeasure do we propose next, and how will we measure success?

Artifacts to bring

  • Short trend charts (2–6 weeks).
  • Recent incident summaries or defect samples.
  • Current action tracker with owners and statuses.

Outcome expectations

At the end of the weekly huddle you should have: a prioritized next step for at least one recurring problem; a list of verified actions with owners and dates; and a clear set of metrics to watch next week.
